Bihac's history dates back to the medieval period, with the old town of Ostrovica being a significant medieval building located above the left bank of the Una river. The fortress was strategically placed, connecting the northern and southern parts of the Una valley, offering breathtaking views of Kulen Vakuf.

In 1262, Bihac was proclaimed as a royal free city and placed it under the direct authority of the Hungarian throne, with all rights and privileges pertaining thereto, which ensured its ability to develop completely independent from the political powers of local lords.

In the late 19th century, Bihac was a key location during the Austro-Hungarian annexation of Bosnia and Herzegovina. The city was defended against Austro-Hungarian troops, but eventually surrendered in September 1878.
